IF YOU HAVE FILED (OR WILL BE FILING) A 2018 FEDERAL INCOME TAX RETURN,
YOU MUST BRING IN A COMPLETE COPY AND YOUR 2018 SOCIAL SECURITY
EARNINGS FORM SSA1099 (FOR EACH PERSON APPLYING).
IF YOU DO NOT FILE A TAX RETURN, THEN YOU MUST BRING IN ALL FORMS OF
INCOME INCLUDING YOUR SOCIAL SECURITY SSA 1099, FOR THE YEAR 2018.
IF YOU ARE 100% DISABLED, PLEASE BRING IN CURRENT (NO OLDER THAN THREE
YEARS) PROOF.
IF YOUR PROPERTY IS IN THE NAME OF A TRUST, THEN YOU MUST BRING IN A
COPY OF THE COMPLETE TRUST FOR REVIEW.

VETERANS PROGRAMS
IF YOU ARE A VETERAN, YOU MAY ALSO APPLY FOR BENEFITS UNTIL OCTOBER 1,
2019. PLEASE BRING IN PROOF OF INCOME, AS LISTED ABOVE.
